Immune thrombocytopenia purpura (ITP) is a low level of platelets with normal bone marrow. It causes various characteristic features such as red and purple bruises/rashes. ITP is an autoimmune disease with antibodies against several platelet surfaces. It includes formations of bruises (purpura) and Petechiae (tiny bruises) sometimes with bleeding from nostrils. Introductions

Azathioprine (AZA) is an immunosuppressive agent which is associated with the conditions such as Atopic dermatitis, Immune thrombocytopenia (ITP), and Psoriasis. It is helpful in reducing the number of flares in relapsing conditions. Azathioprine comes in the form of a tablet which is swallowed orally. It is helpful in stopping the immune system from attacking the organs. It decreases the activity of the body’s immune system. Azathioprine is helpful in preventing the rejection of the kidney and liver allografts, which get absorbed from the gut and vary from patient to patient. It is a thiopurine which is linked to the second heterocycle, which was synthesized by George Herbert Hitchings and Gertrude Elion in 1957 to produce the 6 MP in a metabolic state. Its chemical formula is C9H7N7O2S. Azathioprine was granted FDA approval in 1968.

Mechanism Of Action

It inhibits purine synthesis, which further produces DNA and RNA. Azathioprine is converted into the tissues to 6-MP, which gets converted to 6 Thioguanine in addition to the amino group. The 6 MP and 6 Thioguanine are conjugated with ribose and then form a nucleotide thioinosinic acid and Thioguanylic acid and form the building blocks of DNA and RNA.

  • The nucleotide is newly synthesised DNA and halts replications.
  • Nucleotides inhibit the enzymes which are involved in purine biosynthesis and GPAT inhibition through negative feedback known as Product inhibition.
  • The nucleotide is phosphorylated to the triphosphate form which binds to GTP binding and blocks the synthesis of protein Bcl-xL and sends the activated T cells and mononuclear cells into apoptosis.

Pharmacokinetics

The drug is inactivated in the liver, Azathioprine is a prodrug which is not an active drug but it gets activated in the body. Is an immunosuppressive agent which functions through the modulation of Rac1 to induce T-cell apoptosis. Azathioprine is 30% which gets bound to proteins in human serum albumin in the circulation. The end product of the azathioprine metabolism is thiourea acid and various hydroxylated purines which are further excreted via urine. The half-life of azathioprine is approximately 5 hours.
